Take Your Health Risk Assessment

How do I access the Health Risk Assessment to take it?

1. Log into Working Well here, but click the green Login button on the right. If you are not currently registered, see instructions at the bottom.

2. Click on the My Place link in the left had navigation bar.

3. Click on the “Complete your HRA” link found in the overview tab of “My Health Risk Assessment” of your screen.

If you are not a registered user of Working Well, you can sign up by following these steps:

1. Have your employer give you your Working Well employee number – you can get this from your Human Resource Department.

2. Click the “Register Now” link under “Get Inspired” Members sign-in section using the “Login Link” under “Get Inspired”. You will create a user name and password and will be given immediate, secure access to your personal information.

Don’t forget to try the other online tools and features available through the Working Well.
